Product Description
Tri-Heart Plus (Ivermectin/Pyrantel) Chewable Tablet for Dogs up to 25 lbs is an FDA-approved prescription tablet with 68 mcg ivermectin and 57 mg pyrantel pamoate. It prevents heartworm by clearing Dirofilaria immitis larvae within 30 days of exposure and treats roundworm and hookworm infections. The beef-flavored chewable is given once a month and is suitable for small breeds and puppies 6 weeks and older.
Benefits
- Heartworm Prevention: Clears Dirofilaria immitis tissue-stage larvae within 30 days of mosquito exposure, stopping them from developing into adult heartworms.
- Roundworm Treatment and Control: Targets and clears adult roundworm infections in small dogs and puppies, covering both Toxocara canis and Toxascaris leonina species.
- Hookworm Treatment and Control: Clears three hookworm species in small dogs every month, covering Ancylostoma caninum, Uncinaria stenocephala, and Ancylostoma braziliense.
- Beef-Flavored Palatable Chewable: Most dogs took the tablet on the first try in acceptability trials, making monthly dosing easier for owners of small or picky breeds.
- Broad Breed Safety: Proven safe at the recommended dose across many dog breeds, including pregnant females, stud dogs, and puppies from 6 weeks of age.
- Compatible with Veterinary Products: Safely used alongside flea collars, shampoos, antibiotics, vaccines, and steroids in heartworm prevention programs during clinical trials.

How to Use
Dosage and Administration Instructions
- Prescription Required: Tri-Heart Plus is federally restricted and must only be used under a licensed veterinarian's order with a valid prescription in hand.
- Small Dogs Monthly Dose: Give one blue-box tablet by mouth once a month at a minimum of 2.72 mcg ivermectin per lb and 2.27 mg pyrantel per lb.
- Administration Method: Pop one tablet out of the blister card and offer it by hand, tuck it into dry food, or place it at the back of the mouth.
- Encourage Chewing: Give the tablet in a way that gets the dog to chew it; for dogs that gulp food down, break it into pieces first.
- Confirm Full Consumption: Watch the dog for a few minutes after giving the dose to make sure the whole tablet was eaten and nothing was spit out.
- Consistent Monthly Schedule: Give on the same date each month, starting within 30 days of first mosquito exposure and ending within 30 days of the last.
- Missed Dose: If more than 30 days have passed, give the tablet right away and continue the regular monthly schedule from that point on.
- Switching Products: When switching from another heartworm product, give the first Tri-Heart Plus dose within 30 days of the last dose of the previous one.
- Pre-Treatment Heartworm Test: Test all dogs for heartworm before starting; any infected dog must be treated to clear adult heartworms and microfilariae first.

Additional Information
Precautions
- Not Effective Against Adult Heartworms: This tablet clears larval-stage heartworms only; dogs with existing adult infections must receive adulticide treatment before starting this program.
- Microfilariae Reaction Risk: Dogs with circulating microfilariae may show mild reactions like short-term diarrhea; this tablet does not clear microfilariae from the bloodstream.
- Collie Breed Sensitivity: Collies and Collie crosses can be more sensitive to ivermectin at high doses, but no toxicity was seen at the recommended 6 mcg/kg dose in safety studies.
- Choking Risk: Dogs that tend to swallow treats whole may choke or develop an obstruction; break the tablet into smaller pieces before giving it to these dogs.
- Minimum Age Requirement: Only give Tri-Heart Plus to dogs 6 weeks old or older; puppies younger than 6 weeks should not receive this tablet without direct veterinary guidance.
- Keep Away from All Animals: Store in a secure spot well out of reach of all pets to stop any animal from accidentally getting into the tablets.
Possible Side Effects
- Vomiting and Diarrhea: Vomiting or diarrhea within 24 hours of dosing was seen in 1.1% of doses in clinical trials and is among the most commonly reported reactions.
- Lethargy and Anorexia: Tiredness and reduced appetite have been reported in dogs after dosing based on post-approval adverse event experience with ivermectin/pyrantel tablets.
- Neurological Signs: Seizures, unsteady movement, and muscle tremors have been reported after product approval and need immediate veterinary attention if they appear after dosing.
- Hypersalivation and Pruritus: Excessive drooling and itching have been reported after use; keep a close watch on your small dog in the hours following each monthly dose.
Storage Information
- Controlled Room Temperature: Keep this tablet in a spot that stays between 59°F and 86°F (15-30°C), away from heat and moisture at all times.

Frequently Asked Questions
Q1: What parasites does Tri-Heart Plus Blue Box protect against in small dogs?
Ans: Tri-Heart Plus for dogs up to 25 lbs prevents heartworm disease (Dirofilaria immitis) and treats and controls three hookworm species (Ancylostoma caninum, Uncinaria stenocephala, Ancylostoma braziliense) and two roundworm species (Toxocara canis, Toxascaris leonina) in small dogs and puppies with one monthly chewable tablet.
Q2: Is Tri-Heart Plus the same as Heartgard Plus for small dogs?
Ans: Yes, Tri-Heart Plus contains the same active ingredients, ivermectin and pyrantel, at the same dose levels as Heartgard Plus. Both products are FDA-approved and provide the same protection against heartworm disease, roundworms, and hookworms. Tri-Heart Plus is an FDA-approved alternative that contains an identical formulation.
Q3: What is the minimum age for using Tri-Heart Plus heartworm prevention tablet in dogs?
Ans: Tri-Heart Plus can be used in dogs and puppies that have turned 6 weeks old. Any puppy younger than this should not receive the tablet. Before starting, all dogs must also be checked for an existing heartworm infection by a licensed veterinarian.
Q4: Is Tri-Heart Plus ivermectin pyrantel chewable safe for pregnant or breeding small dogs?
Ans: Yes. Ivermectin/pyrantel has shown a wide margin of safety at the recommended dose in dogs, including pregnant bitches, breeding males and females, and stud dogs. In clinical trials, the combination was also safely used alongside many commonly administered veterinary products.
Q5: What should I do if my small dog misses a dose of Tri-Heart Plus monthly heartworm tablet?
Ans: If the 30-day dosing interval is exceeded, give the tablet immediately and resume the regular monthly schedule from that point forward. Delaying a dose increases the risk of adult heartworm development; prompt resumption of the schedule minimizes this risk. Never give two doses at once.
